The Dietitian counsels patients and/or family members on special nutrition requirements and participates in Quality and Performance Improvement activities. The staff member will be able to demonstrate the knowledge and skills necessary to provide services appropriate to the age of the patients served. The employee will be able to access data reflective of the patient’s status and interpret the information needed to identify each patient’s requirements relative to his or her age and specific needs and to provide services as described in the department’s policies and procedures. Additionally, the Food and Nutrition Services Clinical Dietitian performs all other responsibilities as assigned.

Work with a team of clinicians in developing and monitoring short and long-term care plans. Assess the nutritional status of the patients and develop an individual diet for each patient. Provide individualized nutrition education and counseling. Monitor nutritional status, laboratory values, dialysis kinetics, and response to diet, evaluate outcomes and make modifications/recommendations. Participate in the care center’s QA/CQI process.
